What TDSB forms need to be filled out before coming to Mono Cliffs Outdoor Education Centre?

Agreement to participate form


TDSB now has forms 511C and 511E translated into Portuguese, Somali, Spanish 511SArabic, Farsi, Punjabi, Urdu 511TChinese, Korean, Russian, Vietnamese 511U, and Hungarian 511V.  Please use these to help parents fill in the English version of the form so that it is accurate when you arrive.


What do students need to bring to Mono Cliffs?
Lunch for the first day (lunch needs to be ready to eat and contain no peanuts, nuts or nut products).

Health reminder:

If you indicated on the Medical Form (511C) that your child needs to bring
an Asthma Puffer or EpiPen with them, the puffer or EpiPen MUST be brought to Mono Cliffs.

Where do students and staff eat?

Our kitchen staff does a great job of serving those students with a variety of dietary needs.  If a student has any dietary restrictions, make sure that they are clearly indicated on the their health form, so that our chef can be ready to serve your child. The kitchen staff can provide meals to suit a variety of special dietary needs pertaining to religious, medical or health reasons. If in doubt, please feel free to contact the Centre directly to make sure that your needs are met.

bringing their students to Mono Cliffs. These programs have curriculum
