ORIGINAL DOCUMENTS REQUIREMENT

The Toronto District School Board requires schools to review CURRENT and ORIGINAL documents as listed below at the registration appointment. Photocopies are not accepted in lieu of originals.  By discretion of the school Principal or Vice-Principal, additional documentation may be requested on a case-by-case basis.  Registration appointments may be rescheduled to a future date if documents are incomplete.

** All documents must be originals and not self-printed**

  1. Verification of Primary Home Address – TWO (2) different items from the list below and note that both documents to show the name of the parent/legal guardian and primary home address.

    *Note: Driver’s Licence is NOT accepted*
    • First of two items:  most recent correspondence from Canada Revenue Agency
    • Utility bill (e.g. hydro, gas, water)
    • Cable TV, internet, home phone bill
    • Insurance policy (home, tenant, auto or life) 
    • Mortgage statement
    • Rental or lease agreement or deed
    • Current purchase and sale agreement
    • Property tax bill or notice of tax assessment
    • Original correspondence from a government agency or legal clinic may be accepted as a second proof of address document
  2. Verification of Student Legal Names, Age, Date of Birth and Canadian Citizenship or Eligible Immigration Status - ONE (1) item.
    **Students who do not hold Canadian citizenship or PR status, please provide the student's home country passport with the date of entry into Canada.**
    • Birth Certificate
    • Passport
    • Canadian Citizenship Card or Certificate
    • Permanent Resident Card/Confirmation of Permanent Residence (PR)
    • Refugee Claimant Documentation
  3. Most recent Report Card, Secondary School Credit Counselling Summary and Transcript listing all interim grades and courses earned to date in the current school year, as applicable.
  4. Special Education information, I.E.P. and other documentation, if applicable.
  5. Ontario Health Card or other health insurance coverage for the student.
  6. Business Telephone Numbers and Email Addresses for parents/guardians.
  7. Two (2) additional Emergency Contact Names and Telephone Numbers other than parents/guardians.
  8. Student Registration Form.
  9. Guardianship or Custody Documents, if applicable.  If non-applicable, both parents/legal guardians are assumed to have joint custody of the student (age under 18 years old).
  10. Canada Work Permit & Passport of parent/legal guardian, if applicable.
  11. English & Math assessment package from the Newcomer Reception Centres, if applicable.   Secondary students new or returning to Ontario, after a period of absence for one or more school year, may contact the West End Reception Centre to schedule an assessment appointment.  The West End Reception Centre is located at 777 Bloor Street West, 4th floor, Toronto, ON M6G 1L6 (south side of Bloor St. at Christie subway station), Tel: 416-393-0542.
