There are a number of forms that need to be reviewed. All the forms are extremely important for the safety and wellbeing of students and staff. Please complete these forms and bring them to your Home Form teacher on Tuesday, September 3^rd, 2019.

· Medical Information Form 511E (yellow) - Provide important medical information for your child

· Central Technical School Code of Conduct (white) (NOT INCLUDED IN THE MAILING)

· Student Accident Insurance Letter (white)

· Student Fees Information Sheet (green)

· School Cash On-line (blue)
