TDSB Health Screening

Important Information for Returning to School - Health Screening

TDSB Student Health Pass Form

Each day, you must complete the health screening with your child and then sign and date the Health Screening Paper Pass.  This should be brought to school each day with your child.  Your child can show it to their teacher, while waiting to enter the building in the morning.  Once the teacher has verified that all students have completed the health screening, students will put the form back in their backpack and the class will enter the building.

Each teacher will have extra copies of the health pass available on the first day, if you are unable to print it at home.

Please note, if a student has not completed the screening before coming to school, parents will be called.  If a child completes the screening at school and is not cleared, parents will be called and will be required to pick up their child.

The most important thing you can do to prevent the spread of Covid 19 is to screen your child every day, and keep them home if they have any of the symptoms listed on the screening tool.
