• Toronto Public Health – Immunization Form or on-line at Toronto Public Health
• TDSB Welcome to Kindergarten Booklet
• Kindergarten FAQs
• Starting Kindergarten Flyer

Please note that families are also requested to provide the following documentation for registration:

• Birth date verification
• Country of Birth/Status in Canada verification
• Ontario Health Card or Private Insurance Policy Number
• Two documents to verify home address

(ie telephone bill, bank statement, income tax form, house/auto insurance)
