For Kindergarten, Primary and Junior Grades at Perth

If you are registering a student who is coming from a different school board, click HERE to register online.
You can register online here for Kindergarten.
OR if your child is coming to Perth from another TDSB school, you may register online in the registration portal and email the office staff to let us know which TDSB school your child is transferring from so that we can contact them to request their school records.
When you have filled out the registration form, you will need to show the following items to the Perth office:
- Child's Birth Certificate and/or Passport
- Child's Health Card
- Child's Immunization Card or list of immunizations dated and filled out by the child's doctor
- Two items which act as Proof of Address for you: telephone bill, cable bill, tenancy agreement, and/or driver's license
- If your child is in Kindergarten to Grade 2, please fill out and print the Developmental History Form.
