All school forms are sent at the beginning of the school year to parent emails that are on file with the school.  Completing the information on these forms provides permissions and guidance for staff in regard to your child.  The forms entitled 'Start of the Year Forms'.  These forms include but are not limited to:

• Permission to Participate in Walking Excursion Form
• TDSB Student Media Consent Form
• Permission to Leave School in Severe Weather Conditions
• Intramural Activities Consent 
• Code of Conduct and Online Code of Conduct Acknowledgement
• Medical Forms

If you do not receive the electronic forms in your email by the end of the first week of school, contact our main office for assistance.  We ask that these forms are completed as soon as possible.
