YOU WILL NEED THE FOLLOWING TO REGISTER:

1. A Credit Counselling Summary/Status Sheet from your current/most recent school;
2. A copy of your IEP, if you have one;
3. A parent/guardian if you are under 18.

We'll need to see the following original documents:

1. Proof of birth- Canadian birth Certificate, Passport, or Permanent Residence card;
2. Two proofs of address- Driver's License, utility bill, phone bill, lease, bank statement, pay stub;
3. Ontario Health Card.
