Planning Tools and Services:

  1. Summer Well-Being Guide: This is a guide to help plan for summer activties. It is offered in multiple langauges. 

  2. Information Letter for Parents/Guardians: This letter outlines steps and resources for support networks of students with complex needs. 

  3. Passport Program Guide: This link will open a booklet about the Passport Program. 

  4. Community Services for Adults with Disabilties: This is a list of agencies for families.

  5. Parent Toolkit:  A booklet with information and links to resources to assist in planning your student's life after Maplewood.

  6. Respite Services Guide: A Guide for caregivers of children and adults with disabilities in the city of Toronto.

  7. Planning for Life After High School: This set of presentation slides has a lot of information to prepare families, guardians and students for life after Maplewood HS.

  8. Culturally Relavant Resources: This is a set of slides with useful information and connections to community resources.

  10. Journey to Adulthood Checklist: This is a list for parents provided by Autism Ontario.

  11. ConnectAbility: Resources for adults with developmental disabilities. 

  12. AccessOAP: This is funding for students under 18 with a diagnonsis of autism.

  13. Hope 4 Parents: Support for parents and guardians who are troubled by the behaviours of a teen or young adult in their life.

  14. Ontario ID Card: For students that will need government ID, but are not able to obtain a drivers liicense. 

  15. City of Toronto: This link goes to a list of services available in the city of Toronto for adults with disabilties.
