To register your child please contact the Office at Guildwood Jr. P.S. at 416-396-6295 to set up an appointment. Your will need to bring the following documentation:

Two proofs of address (utility bill, property tax statement, etc)

Proof of birth (Canadian birth certificate, Canadian passport, Canadian baptismal certificate)

Health Card

Immunization record
