Student Information Forms: It is imperative that parents/guardians complete "The Start of The Year Form" . As you read through the form be sure to update/confirm medical information, address, phone numbers, emergency contacts and to authorize participation in certain activities. Guardians were emailed these (individualized) forms for their child/children through “Permission Click” on September 4th. The forms are fillable and can be electronically submitted once completed. Please ensure this is completed by the end of the week.
