EMA Excursion Form:

The TDSB offers a NEW help document on completing the EMA. Each program also has information around the curriculum goals and itinerary listed. Please consult the specific program on our website for the required information.

The classroom teacher is required to bring the Student Excursion Report by Class Code. This report can be obtained from your school’s Office Administrator. It is in Report Generator listed as ’Student Excursion by class’.

If a student who has a Diabetes Management Plan in place is attending Forest Valley, as per TDSB policy, the teacher must ensure appropriate supplies and Pocket Emergency Card are brought on the trip.

Transportation:

Schools are responsible for booking and paying for a school bus. When booking a bus, please arrange for it to arrive at Forest Valley as soon as possible in the morning and depart from Forest Valley at 2:00.

Payment:

Please give your trip money to your OA. User Fees, which are based on your school’s placement on the Learning Opportunities Index, are charged for the number of students that arrive on the day of the trip. We will do a Journal Entry Budget Transfer and include your Office Administrator and Principal on the email to Accounting.


Please Note - new cancellation policy.  Program bookings cancelled within a two week time frame will be charged a $300.00 cancellation fee (through a journal entry to the school).
